ORIGINAL:

I've put together what I believe to be the most accurate and comprehensive movie trailer rating template available online. It includes green band, red band, and yellow band screens, with alternate text, editable text, all ratings (G, PG, PG-13, R, NC-17), accurate MPAA logo, and accurate layouts. I hope you enjoy. 

The Illustrator template is more fully featured, but I've included a Photoshop template that is good as well.

I wrote an article titled "How the Movie Rating Screen was Designed" that goes in-depth into the different elements of the layout. It also includes links to the fonts used in the templates.
